Rivne city hospital has crossed its 175-year anniversary and at present is a multifunctional institution with great history and glorious traditions. The personnel of the hospital is characterized by the spirit of innovations which looks with optimism into future.
On 805 beds are functioning 17 departments (7 – therapeutical, 10 – surgical).
Neurosurgical, goathic-facial surgery, 2 infectious (for children and adults) units have the status of regional ones.
In the hospital there are centres of specialized aid:
- arthrology and recovery treatment;
- microendoscopic otolaryngology surgery and cosmetic surgery;
- hepatological.
During last 10 years it had been possible to alter almost completely the hospital style, to renew material and technical basis, to strengthen the staff potential which resulted in introducing a wide range of present-day medical know-how.
Today in the hospital are functioning: a computer tomograph, an angiograph, a lithotriptor, a sinusoscope, a momograph, laporoscopic techniques, monitors for dynamic observation over heavy patients, present-day inhalation anesthesia and roentgenological equipment, laboratory devices.
The hospital has 236 doctors, including 4 candidates of medical sciences, eight merited doctors of Ukraine.
Annually the hospital provides aid to 24-25 thousand patients, over 13 thousand operations are being carried out, over 300 patients are treated for myocardium infarct, over 400 – for acute disturbance of brain blood circulation, up to 3000 for traumas.
The visiting card of the hospital is an admission diagnostic department with a complete set of necessary premises (examination rooms, a manipulation room, a dressing room, 2 operating rooms, a laboratory, a roentgenogram room, an ultrasonic diagnosis room, sanitation premises). The department has 7 doctors-specialists on duty day and night which permits to render urgently both consultative and also necessary qualified medical aid. The deparment admits daily up to 130-150 patients.
The city hospital does all types of operative intrusions on abdominal cavity organs, laparoscopic operation methods have become widespread, proper attention is paid to the world tendency in surgery to microinvasive treatment methods. The range is expanded of operations on stomach and duodenum both of resections and also of organ preserving ones; on bile conduction tracts – reconstructive and plastic, on pancreas – with cysts and pancreatitis; under tumors of large intestines.
The level and quality has substantially increased of operations on kidneys, ureters and a urinary bladder. The splitting is carried out of stones with the help of contact lithotripsy. Potentials are expanded of a traumatological department where operations are conducted:
- osteosynthesis of pelvis bones;
- stabilization of fractures of proximal and distal sectors of a hip bone by dynamic screws (DHS and DCS correspondingly);
- stabilization of fractures of diaphyses of long tubular bones by intramedular blocking pin.
The neurosurgical department carries out operations on account of traumas of cerebrum and spinal cord, low-level invasive endoscopic intrusions under the pathology of intercostal disks, the setting of stabilizing spine systems, of voluminous processes, the plastics of posttraumatic bone defects and traumatic injuries of peripheric nervous system, liquorshunting operations, diagnostic selective angiography, Doplerography of neck vessels and of extracranial head vessels.
In the centre of arthrology and recovery treatment doctors have mastered techniques of total cement and cementless endoprosthetics of a thigh joint using endoprostheses manufactured by leading American and European firms – BIOMET, Zimmer, Stryker, DePuy. Techniques are mastered od subtotal endoprosthetics of a knee joint. At present the centre had conducted over 1450 arthroscopic operations, 118 transplantations of thigh and 5 knee joints. The basis of rehabilitation treatment is a balneological complex with a swimming pool, a horizontal underwater traction, a circular shower,a hydrolaser booth, a biospectral sauna.
Rivne city hospital has acquired large experience in treating the acute infarct of myocardium, In 1980 the department had been organized for patients with myocardium infarct, since 1992 – wards of intensive care therapy for acute myocardium infarct. Practically during all these years the most recent techniques are used of treatment (including thrombolysis) which resulted in the possibility of keeping the lowest lethality from acute myocardium infarct in the western region. At present the treatment of acute myocardium infarct is impossible without carrying out intervention treatment methods ( primary angioplastics and stent implantation). The hospital had introduced into action from 2004 the angiograph which had given the opportunity to conduct coronorography and stent implantation
for patients during pre-infarct and post-infarct periods. The acquired experience gives the possibility to render patients with myocardium infarct proper aid by methods of intervention cardiology.
The centre of intervention cardiosurgery is a rather young structure in the hospital but by May 2008 it had conducted:
- coronarographies 500;
- coronary stent implantations 275;
- angiographies 100;
- setting cava-filters 20;
- surgical intrusions on peripheric vessels 81;
- cerebral angiographies 4;
- implantations of permanent pacemakers 14, temporary 25.
The administration of the hospital taking into account the spreading of vessel pathology in 1998 had opened the department of vessel neurology with wards (12 beds) of intensive care which had given the possibility to approach to and to improve the urgent qualified doctor’s aid for patients with acute disturbances of brain blood circulation, to provide the quality supervision over patients, to develop issues of succession in cooperation with a neurosurgical department. These measures contributed to decreasing the death rate, the disableness of patients with this pathology.
The department of anesthesiology and intensive therapy is one of the best in the hospital both in the qualification of the personnel and in the equipment available. The department staff consisting in the beautiful symbiosis of experience and youth is able to treat patients with the most complicated pathology: acute politrauma, insults, acute pneumonias, pancreonecroses, septic states, etc. The department is equipped with the most up-to-date respiratory devices, including “Hamilton”, bronchoscopes, monitors, devices for plasmapheresis, a modern laboratory.
Besides, the mighty operational basis of the hospital with available present-day anesthetic devices “Megamed” and using modern anesthetics gives the possibility to carry out annually over 4000 common anesthesias.
In connecton with the spreading of the pathology of otolaryngological organs, in particular the increased pathology of ear, of outer nose and additional nose cavities and the advancement in the world and in Ukraine of plastic and reconstructive surgery on the basis of the otolaringological department a Centre was created of microendoscopic surgery for otolaringological organs and cosmetic surgery where such volume of operative intrusions is carried out: endoscopic surgery of ear, of additional nose cavities and of outer nose, hearing improvement operations for middle ear, the correction of outer nose (closed rhinoplastics), all kinds of rejuvenating operations on the face, the correction of forms of mammary glands, of stomach and also of a figure.
At present the situation with chronic virus hepatitis is quite justfully characterized as a “concealed epidemy” which forced to look for ways of solving this problem. In 1998 a hepatological centre had been organized. The creation of the centre provided for a scientific approach to carrying out the diagnosis and treatment of hepatites. By the IFA method markers are determined of virus A.B.C. hepatites, hepatotropnic viruses, markers of autoimmune hepatites. All patients getting antivirus treatment are necessarily determined concerning not only hepatites markers but also as regards their subtypes and virus loading by the method of polimerasic reaction.
The treatment of patients with chronic virus hepatites is carried out in accordance with advanced technologies by present-day antivirus medicines.
Large aid in diagnostic process is rendered by the hospital laboratory equipped with modern devices: a biochemical analyzer “Technikon 100”, a microanalyzer, a hemocoagulometer, an immunoferment analyzer, etc. Per day the laboratory conducts up to 3500 thousand laboratory studies. The laboratory staff works constantly at renewing and expanding the range of present-day methods of laboratory studies and cares for the work quality.
The clinic carries out great preventive work on revealing arterial hypertension, tuberculosis, oncological pathology, infectious diseases. The work is properly organized on the sanitation of dispensary patients group and particularly extreme forms of diseases.
The clinic has 2 day-time facilities for therapeutic, neurological patients and those with skin diseases.
During 2003-2007 the tendency is kept for improving and stabilizing indices of the initial transfer to disability, in particular of persons with capacity to work; indices are improved for rehabilitation of disabled persons.
The hospital adminisration contributes to increasing the level of the qualification for medical cadres both at native scientific bases and at best European clinics.
During only last 10 years 21 doctor had been trained in Poland, Germany, Hungary, Great Britain, Italy.
Today the hospital has 65% of doctors with the 1st and highest category, 70% of paramedical personnel are certified.
Thye hospital often receives doctors from Poland, Austria, Gemany, Canada, USA, Russia, Byelorussia; scientific practical conferences are practiced with their participation.
The hospital lives with not only daily troubles but also pays attention to organizing the recreation: the study of our country history visiting historical and memorable sites of Ukraine, carrying out physical health measures and amateur activities. Clinic №1 is a structural subdivision of CCH in Rivne and provides service to 367.7 thous. population. Its structure includes also 5 first-aid and 1 medical stations. The annual numer of attendences is about 460 thous. The hospital staff comprises 244 workers, including 75 doctors. The clinic has 30 therapeutic units; city traumatology centre is functioning day and night; specialists of narrow profile carry out the reception: endocrinologists, oculists, surgeons, traumatologists, otolaringologists, urologists and also a cardiologist, a rheumatologist, an infectionist, an immunologist, an oncologist, a pulmanologist, a nephrologist. The hospital has 2 day-time facilities: therapeutical for 20 beds and also for dermatological patients for 35 beds. Among doctors 56% possess the highest attestation category, 28% - the first and 15% - the second . Diagnostic and physiotherapeutical subdivisions are joined with hospital departments.

The admission diagnostic department is situated in the surgical building of CCH. The department functions day and night. Patients are divided into flows: planned ones and urgent ones. All urgent patients are examined maximally, if necessary the medical urgent aid is rendered in full volume. In night besides the therapeutist on duty in the hospital there are on duty: a neurosurgeon, a surgeon, a cardiologist, a neuropathologist, a resuscitation specialist, an infectionist. In the admission department day and night function: a laboratory – where it is possible to make a general blood test, a general urine analysis, a biochemical blood analysis; a roentgenography room, a dressing room, two operation rooms. Day and night a pharmacy is functioning.
The admission department is in due time supplying hospital patients with blood and blood preparations. If necessary via the admission department specialists from other regional institutions are summoned.
The admission department has a room for sobering up persons in the state of alcoholic drunkeness, if necessary such people are given disintoxication therapy. The admission department has day and night a sanitary car which is used for transporting blood, blood preparations, consulting doctors. During the night the admission department is under protection.

The department of anesthesiology and intensive therapy is located on the basis of the central city hospital in Rivne in a surgical building and includes 12 beds. In the department staff are 19 doctors, including 7 with the highest category; 13 anesthesists, including 9 with the highest category; 20 medical nurses, 8 – with the highest category. In the department work 2 merited doctors of Ukraine, including the head of the department – Korolyuk V.O. The department is equipped with the up-to-date native and foreign devices, including monitoring complex for 12 persons, narcosis respiratory equipment, including AHEMAT-MIHI and MIHI-OKCIBEHT. The department is supplied with the most up-to-date respiratory equipment, the only one in Ukraine type (premium class) HAMILTON G5. It should be noted that taking into account the application of the said technical equipment in the laboratory and department the number of patients increases annually from 960 to 1129 people. The major number of patients constitute the post-operation group of patients amounting to 50%. The second place is taken by craniobrain traumas 7-23%. Suicidal attempts – 4,2%. Acute disturbance of brain blood circulation – 3,9%. Poisonings – 4,5%. 75,8% are patients with the disturbance of vital functions of the organism. 24,2% is the group of patients in need of intensive care and treatment. 79,5% patients are transferred to profile departments. 8% are discharged home, they are mainly patients with easy forms of poisoning. Anesthetical aspect of work of the department includes 86-88% of all surgical intrusions in the hospital by anesthetical support. The department provides for 3354 operative intrusions, including 50,4% urgent anesthesia.In the department functions the room for extracorporal metnods of detoxication:

The cardiological department is located on the 1st floor of the hospital surgical building. The total area– 320 sq. m., 15 wards, 3 wards of improved type with all communal benefits. The total number of beds – 40.
The department includes the block intensive therapy for 6 beds and resuscitation hall. The head of the block of intesive care – Pomohov Yuriy Mykolayovych, doctor-cardiologist of the highest category.
The department renders specialized medical aid to patients with acute myocarditis infarct and its complications having proper technical support.
The ward of intensive care has the room with 6 beds and resuscitation hall.
Day and night there is a cardiologist on duty with medical nurses and a practical nurse.
The department renders an urgent medical aid to patients with myocarditis infarct and its complications, to patients with pulmonary edema, with disturbances of heart rhythm, with thromboembolism of lung artery.
An urgent coronarography is carried out and also stent implantation to patients with myocarditis infarct, with non-stable and stable stenocardia.
The department carries out daily monitoring AT, ECG, oxylenotherapy, medicines infusion by intravenous drops and also with the help of infusomath. Patients with myocarditis infarct cardiogenic shock are given contrapulsation.
Patients with myocarditis have systemic thrombolysis (streptokinase, aktilizis).
The department for patients with complicated disturbances of heart rhythm and disturbances of hemodynamics carries out urgent and planned cardioversions. If necessary are taken ECG via esophagus, a temporary cardiostimulator is set for patients with myocarditis infarct complicated by AV-blockade and arhythmic shock.

The infectious department for children for 30 beds is a structural subdivision of the multiprofile city hospital. The department is completely boxed which gives the possibility to hospitalize patients with different infectious pathology.
The children’s infectious department renders in-patient and out-patient medical aid to children with infectious diseases residing on the territory of Rivne and region.
The department has two doctors who had obtained specialization in children’s infectious diseases. All doctors are attested and have the highest category.
Annually the department treats over thousand little inhabitants of the city of Rivne and the region, including 30% - children of the first two years of life.
Under conditions of boxed wards are treated patients with highly contagious diseases (measles, rubella, scarlet fever, epidparotitis), there is a separate box in case of particularly dangerous diseases. Doctors provide for day and night consultative diagnosis aid, take an active part in the eradication of epidemic outbreaks of such infectious diseases as diphtheria, salmonellosis, dysentery, virous hepatitis A which occur in different corners of the region.
In conditions of the department it is possible to conduct the diagnosis of the wide spectre of infectious diseases by the method of IFA and PCR.

NEUROLOGICAL DEPARTMENT
FOR PATIENTS WITH DISTURBANCES OF BLOOD CIRCULATION

The grounds for creating the “vascular” department had become the growth of CVP and the necessity to change qualitatively the rendering of aid to patients with brain insult.
The department is designated for 60 beds, including 12 for particularly heavy patients. During a year, on the average, the department treats 1500-1600 patients, including 450-500 with brain insults. The department also admits patients with chronic insufficiency of brain blood circulation, carries out rehabilitation treatment courses.
The department operates day and night under urgent regime all over the year.
The department is completely staffed with senior and junior medical personnel, doctors. In some recent years the hospital introduced and widely uses the method of Dopplerographic investigation both of extra- and intracranial vessels. With the objective of much wider research the department specialists use the possibilities of Rivne regional clinical treatment diagnostic centre and also magneric resonance therapy on the basis of Rivne regional hospital.
The department has in its staff 1 merited doctor of Ukraine and 1 candidate of medical sciences. All doctors of the department have the highest attestation category, possess the technique of conducting lumbar punctures which are carried out by certain indices with the aim of studying spinal marrow liquid.
The treatment of patients in the department is carried out in a comprehensive way: by medicamentose means, physiotherapy, remedial gymnastics, phytotherapy, psychotherapy, needle reflexotherapy, various blockades are carried out. The department closely cooperates with the neurosurgical department to implement operative methods for treating disturbances of brain blood circulation.

INFECTIOUS DEPARTMENT FOR ADULTS

The department has been functioning as a consistuent part of the central city hospital since 1978 and performs functions of catering for the population of Rivne and the region.
As a regional infectious department it renders treatment consultative aid to patients with heavy infectious pathology from the city and the region. In case of the the emergence of, acute in clinical diagnostic aspect, cases or mass diseases in districts of the region the doctor staff carries out visiting consultative aid by the system of the Centre of the medicine of catastrophes and emergencies.
The doctor staff of department includes 7 doctors among whom:
1 candidate of medical sciences
3 doctors of of the highest category
1 doctor of the 1st category
2 doctors-specialists.
The department is a basic one for the internship of graduates of medical higher educational establishments, for practical training of students of medical colleges and medical universities.
On the basis of the department conferences are held for the paramedical personnel of infectious departments and rooms of infectious diseases of Rivne and the region.
The department performs organizational methodical work on issues of infectology for the region.
In its practical activity the department staff, besides rendering treatment diagnostical aid to infectious patients, as a whole specializes in certain directions of infectious pathology.
In particular:
1. diagnosis and treatment on TORCH-infections (toxoplasmosis, herpes virous infections, etc).
2. treatment of heavy and complicated forms of erysipelas at present-day level
3. diagnosis and treatment of a wide aspect of pathologicak condition of patients with AIDS and HIV-infected ones.
4. department is a basic centre of the regional medicine on issues of fighting against and prevention of particularly dangerous infections (plague, cholera, hemorrhagic fevers, highly pathogenic flu, etc).

In the department are successfully treated patients with different forms of heart ischemic disease, heavy degrees of cardio-vascular insufficiency, primary and secondary types of arterial hypertension, hypertension crises, cardiopathies.
For diagnosis and treatment of hypertensions the department is equipped with the apparatus for monitoring arterial tension.
In CD №2 are carried out treatments of disturbances of heart rhythm as a conservative procedure – taming of tachiarhythmias, by way of medicamentose and electrical cardioversion and by surgery:
– treatment of bradyarhythmia, AV-blockades, syndrome of weakness of sinus node by the implantation of one- and two-chamber heart pacemakers;
– selection and preparation of patients for implanting three-chamber artificial heart pacemakers, cardioverters-defibrillators;
– control over work of artificial heart pacemakers (primary and reprogramming) with the help of programmers of various firms, control over efficiency of treatment, carrying out Holter monitoring.
Wards are equipped for treatment and care for patients with disturbances of heart rhythm with the possibility of near-bed monitoring of ECG and also by defibillators.

As a constituent part of CCH the department has been functioning since 1971.
The department has 50 beds: including 30 beds for treating patients og therapeutical profile, 20 beds for treating patients of rheumatical profile. For diagnosing patients are used laboratory and instrumental methods, computer tomography, ECHO CS, Dopplerography, USD, FGDS, colonoscopy, Roentgenological studies of locomotive apparatus, immunological blood test.
For hospital treatment are admitted patients with diseases of kidneys (pyelonephrites, glomerulonephrites, diabetical nephrooathies), allergic diseases, arterial hypertensions of unclear genesis, joints (deforming osteoarthrosis, rheumatoid arthritis, reactive poliarthritis), spine (ankylosis spondiloarthrosis), systemic diseases of connective tissue, vasculites, gout, rheumatic disease, poisoning of different ethiology, etc.

Has been functioning in CCH for 65 beds: including 40 – gastroenterological; 25 – therapeutical ones.
The department treats patients with diseases of gastrointestinal tract (chronic pancreatitis, chronic cholecystitis, gastroesophageal reflux disease, chronic stomach ulcer and duodenum disease, intestines disease, chronic gastrites with increase, decreased secretory function, chronic gastrites associated with chelicobacterial infection, chronic hepatites of different origin) and also patients with anemias of various ethiology (posthemorrhagic, iron defficient, B12 defficient and secondary arising at the background of other pathologies), patients with poisoning by toxic substances, oncodiseases (for carrying out symptomatic treatment), and also are hospitalized patients with unclear diagnosis for making it more precise and treatment.
The department has a RRS room, a room for duodenal tubage, a room for phytotherapy.
On the base of the department works the apparatus of hydrocolonotherapy. Hydrocolonotherapy is used for treating functional disturbances of bile, biliary tract, pancreas, thick intestines by way of influencing the motor function of intestines and the reflexive action upon bile, pancreas.
The department specialists develop individual recommendations on dietic nutrition under various diseases and also give recommendations on the corrective dieta for those who are eager to decrease their weight.

The department has 50 beds subordinated to clinic № 1 on the ground floor.
The day patient facility treats patients from cardiological, gastroenterological, pulmonological, otolaryngological, ophthalmonological, traumotological profile, patients with diabetes mellitus and its complications.
For examining patients are used most up-to-date techniques of laboratory and fuctional diagnosis. Besides medicamentose treatment patients receive the course of rehabilitation: massage, physiotherapeutical procedures, classes in a swimming pool, a course of corrective gymnastics. New techniques are widely used in accordance with standards. The medical personnel of the department is highly qualified, caters for patients with attention. The stay in a day patient facility has its own advantages, the patient without separation from the family gets complete course of treatment. Costs of a medical institution with this form of treatment are substantially saved.
The department has three doctors, two with the highest attestation category and one – with the second category.

From 2000 on the base of CCH in Rivne had been set up RRTDHC, one of the first hepatological centeres in Ukraine on permission of Ministry of Public Health of Ukraine.
The hepatological centre renders consultative and hospital aid to patients with various pathology of liver for adults and children.
IFA-diagnosis – the determination of markers of virus hepatites A,B,C, autoimmune hepatites. Blood intake is done to determine RNA VHC, DNA BHB, RNA, Epstein-Barru vituses, CMV, herpeses by method of PCR and also toxoplasmosis, chlamydias both by qualitative and quantitative analysis, the so-called virus loading, the determination of cevuloplasmin with the suspicion for Konovalov-Wilson disease, markers of hemochromotosis.
Dopplerographic diagnosis is carried out for organs of abdominal cavity
Patients with the suspicion for liver cirrhosis have diagnostic biopsy of liver with further pathomorphological research.
The treatment of patients is carried out at the present-day level using standards of quality of foreign colleagues. Antivirus therapy is appointed combined with interferons to patients with chronic hepatites B and C.
Specialized hospital aid is rendered to patients with chronic hepatites of different ethiology and liver cirrhosis.
